Benefits Overview
Adapt Forward offers a wide range of Medical, Dental, and Vision benefits through Brown & Brown. The options vary by state/region. More details on all plans offered can be found in the Benefits Guide and in Employee Navigator once access is granted. Your benefits will be effective on the 1st of the month after your start date. If your start date is on the 1st of the month, your benefits will be effective immediately.

Life Insurance/ Disability
100% Employer-paid life insurance, AD&D, and long-term disability.
Short-term disability provided through MetLife.
Flexible Spending Accounts
Dependent Care Reimbursement & Medical Reimbursement account options available.
Health Savings Account
Adapt Forward will contribute $250.00 semi-annually to an employee’s HSA account.
*Designed for employees enrolled in a High Deductible Health Insurance Plan.
401K Plan
100% Employer match on the first 3% of employee contributions, plus 50% between 3%-5%.
Fully vested after 2 years.
Annual Training Budget
$4000.00 to be used for conferences, schooling, certifications.
Includes travel costs; with management approval.

11 Federal Holidays
15 Annual PTO days accrued at 5 hours per pay period for non-shift workers and 6.25 hours per pay period for shift workers.
